- As of March 1, 2020, we are pleased to announce expansion of clinical services via our Neuropsychological Assessment Clinic – a specialty clinic that provides neuropsychological testing for adults ages 18+ for a variety of neurologic and psychiatric conditions. This change has been included in the revision of our updated referral form. In reviewing our referral form, it is possible that medical staff and/or referring providers will have questions as to which clinic to select in making their referral. Please see below for further instructions.
- For known or suspected dementia, mild cognitive impairment (MCI), or other neurodegenerative conditions, please select “MCI or Dementia Evaluation,” which will indicate a referral to the Alzheimer and Memory Center (AMC). Patients referred to the AMC will undergo a comprehensive workup that may include consultation with behavioral neurology, neuropsychology, geriatrics, psychiatry, family medicine, and social work, and will often require updated neuroimaging or lab work as needed. Cases are discussed at our scheduled multidisciplinary team meetings (MDTS), so that each subspecialty may weigh in with their expertise to create a comprehensive diagnostic and treatment plan for both the patient and their family.
- For all other individuals who require neuropsychological workup (i.e. cognitive testing) please select “Neuropsychology Only” on our updated referral form. Neuropsychological testing is often requested by primary care providers, neurologists, psychiatrists, and other referring providers to understand the etiology, nature, and extent of any known or suspected neurocognitive disorder. Neuropsychological assessment is frequently requested in the diagnosis of ADHD, stroke, TBI, pre-surgical evaluations, autoimmune disorders, neuropsychiatric conditions, etc. and can be instrumental in establishing a cognitive baseline, clarifying diagnosis, and formulating a list of tailored treatment recommendations moving forward. Note: if in the process of referral review, a “Neuropsychology Only” patient requires a more comprehensive dementia workup, they will be referred for the full dementia evaluation through the Alzheimer and Memory Center.
